Volunteer with ADRA

In the event of an accident or illness, I authorise the Team Leader to consent, where it is impractical to communicate with me or my parents (if under the age of 18) to receive a X-Ray examination, anaesthetic, medical, surgical or hospital treatment as may deemed necessary by a licensed physician and/or surgeon.

I have read the ADRA NZ Conflict of Interest/Code of Conduct for employees and related personnel, and I understand its contents. I agree to comply fully with the standards, policies and procedures contained in the ADRA NZ Conflict of Interest/Code of Conduct and ADRA related policies and procedures. I certify that, except as noted below, I am otherwise in full compliance with the ADRA Conflict of Interest/Code of Conduct and any related policies and procedures.
